Plant Sterols and Stanols

The best-validated non-drug LDL intervention.

Plant sterols and stanols are cholesterol-like plant compounds that compete with cholesterol for absorption in the gut, so more cholesterol is excreted. We grade Plant Sterols and Stanols B+ on evidence and A- for worth-it at about $10-15 per month.

What it does

Plant sterols and stanols are cholesterol-like plant compounds that compete with cholesterol for absorption in the gut, so more cholesterol is excreted. They are among the most studied dietary LDL interventions.

What the research says

  • Lowers LDL cholesterol by about 8 to 10 percent at 2 g/day

Evidence: strong. Katan et al. 2003 meta-analysis of 41 trials: 2 g/day stanols or sterols cut LDL by 10%; EFSA confirms 7 to 10.5% at 1.5 to 2.4 g/day. Effects are additive with statins. https://www.heartuk.org.uk/four-cholesterol-lowering-foods/sterols-and-stanols

  • Effect is additive with statin therapy

Evidence: moderate. Adding sterols/stanols to statins lowered LDL further, roughly comparable to doubling the statin dose in trials.

DRAFT: dozens of RCTs and a clear dose-response on LDL; capped below A because no hard cardiovascular outcome trials exist for sterol supplements themselves.

Is it worth it?

A-

DRAFT: one of the cheapest validated LDL reductions available; a genuine statin-adjunct or statin-alternative conversation starter.

Typical cost

$10-15/month

Common form, not a brand recommendation

Dosage contextestablished

1.5 to 2.4 g per day of sterol or stanol esters, divided across meals; effect plateaus above about 3 g.

Safety notes

Generally safe; may slightly lower beta-carotene absorption. Contraindicated in the rare genetic condition sitosterolemia. Take with meals, divided doses work best. This page is information, not medical advice.
