Cocoa Flavanols

The chocolate compound behind the 21,000-person COSMOS trial.

Cocoa flavanols are plant compounds in cacao that support nitric oxide and blood vessel function. We grade Cocoa Flavanols B- on evidence and B- for worth-it at about $15-25 per month.

What it does

Cocoa flavanols are plant compounds in cacao that support nitric oxide and blood vessel function. The COSMOS trial tested a concentrated 500 mg/day cocoa extract, far more flavanols than chocolate can deliver without excess calories.

What the research says

  • Reduced cardiovascular death in a large randomized trial (secondary endpoint)

Evidence: moderate. COSMOS: 21,400 adults, 500 mg/day for 5 years. Primary endpoint (total CV events, -10%) missed significance; secondary analyses showed 27% lower cardiovascular death, 39% in adherent participants. Authors urge cautious interpretation. https://www.sciencedaily.com/releases/2022/03/220316173304.htm

  • Lowers blood pressure and improves flow-mediated dilation

Evidence: moderate. Multiple smaller placebo-controlled trials show meaningful blood pressure and arterial function improvements.

  • May support cognitive function via blood flow

Evidence: emerging. Small trials link flavanol intake to cerebral blood flow and attention measures; clinical significance unclear.

DRAFT: the largest trial in the category with an honest mixed result; primary endpoint missed, secondary signals real but need replication.

Is it worth it?

B-

DRAFT: moderate cost for promising-but-unconfirmed hard outcomes; the blood pressure data alone is decent value.

Typical cost

$15-25/month

Common form, not a brand recommendation

Dosage contextemerging

500 mg cocoa flavanols per day from a standardized extract, the COSMOS dose; not achievable from chocolate.

Safety notes

Well tolerated in trials; trace caffeine in some extracts. Choose extracts standardized to flavanol content, since chocolate and cocoa powder vary wildly. This page is information, not medical advice.
