Supplement
Alpha-Lipoic Acid
Antioxidant with the strongest supplement evidence in diabetic neuropathy
Alpha-lipoic acid (ALA) is a sulfur-containing antioxidant involved in mitochondrial energy metabolism. We grade Alpha-Lipoic Acid B- on evidence and B- for worth-it at about $10-20 per month.

What it does
Alpha-lipoic acid (ALA) is a sulfur-containing antioxidant involved in mitochondrial energy metabolism. It has multiple meta-analyses supporting symptom improvement in diabetic peripheral neuropathy, and is the best-evidenced use case in this batch.
What the research says
Improved diabetic neuropathy symptoms (pain, total symptom score, vibration perception)
Evidence: moderate. 2024 meta-analysis of 8 RCTs: oral 600-1800 mg improved symptom scores; 2025 update: 600 mg/day up to 24 months considered safe
IV ALA improved nerve conduction velocity in diabetic neuropathy
Evidence: moderate. 15-RCT meta-analysis (1,058 patients): 300-600 mg/day IV for 2-4 weeks improved conduction velocities and symptoms, but trial quality was mostly poor
Possible HbA1c reduction
Evidence: emerging. One meta-analysis reported lower HbA1c with ALA, with high heterogeneity; not established
Multiple meta-analyses support symptom improvement in diabetic neuropathy with good safety at 600 mg/day oral. Caveat: many trials are poor quality, and the strongest data come from IV administration, which does not translate directly to oral capsules.
What the experts say
Every position links to its source. We include skeptics alongside supporters.
Bruce Ames
Source, 2002-02-19 His PNAS research showed ALA plus acetyl-L-carnitine reversed age-associated mitochondrial decay and improved memory in old rats; he calls mitochondria the weak link in aging.
David Sinclair
Source, 2025-08-28 Includes alpha lipoic acid in his personal supplement stack, though he rarely discusses it in detail compared with NMN, resveratrol, or metformin.
Is it worth it?
One of the best evidence-per-dollar options in this batch for its indication (diabetic neuropathy); inexpensive and well tolerated at 600 mg/day.
Typical cost
$10-20/month
Common form, not a brand recommendation
Dosage contextestablished
600 mg/day oral is the standard trial dose; IV 300-600 mg/day for 2-4 weeks in neuropathy trials.
Safety notes
Safe at 600 mg/day in trials up to 24 months. GI side effects (nausea, stomach upset) appear at 1,200-1,800 mg/day. May lower blood sugar, so diabetics should monitor glucose and coordinate with their clinician. Avoid in pregnancy due to limited data.
