Serrapeptase

DoseProducts provide 10 to 30 mg (marketed in 'SPU' activity units, typically 40,000 to 120,000 per day), taken on an empty stomach as an enteric-coated tablet, because the enzyme is a protein that digestion dismantles, the absorption problem this whole category shares and rarely mentions.

BasisSerrapeptase is an enzyme with a plausible anti-inflammatory mechanism, fifty years of anecdotal and regional-drug use, and a formal evidence review that says insufficient.

Serrapeptase (serratiopeptidase) is a proteolytic enzyme originally isolated from silkworm gut bacteria, sold for inflammation, swelling, sinus congestion, and pain. We grade Serrapeptase D+ on evidence and D for worth-it at about $12-22 per month.

What it does

Serrapeptase (serratiopeptidase) is a proteolytic enzyme originally isolated from silkworm gut bacteria, sold for inflammation, swelling, sinus congestion, and pain. It is half of this extension's systemic-enzyme pair, with lumbrokinase graded alongside it in this library.

What the research says

  • Scattered small trials suggest anti-edema and mucus effects; a systematic review found the file insufficient

Evidence: weak. Serratiopeptidase has decades of use as a drug in Japan and India and a trial literature to match its age rather than its market size: small studies on post-operative swelling, ankle sprains, carpal tunnel symptoms, and respiratory mucus, mostly short, mostly low quality, several decades old. A 2017 systematic review concluded the evidence is insufficient to support its use for any indication. Nothing since has changed that verdict materially.

Serrapeptase is an enzyme with a plausible anti-inflammatory mechanism, fifty years of anecdotal and regional-drug use, and a formal evidence review that says insufficient. Its bioavailability as an intact oral protein is itself doubtful. D+: the claim has had every opportunity to generate proof and has produced small old trials instead.

Is it worth it?

DWorth it

Evidence (D+) sits far below the leaderboard floor, so the worth-it grade is capped at C+; the derived value lands at D. About $17 a month at common doses. Derived from the October 2026 worth-it model.

Safety notes

Generally tolerated; GI upset and rare skin reactions are reported. As a proteolytic enzyme with fibrinolytic claims, it belongs on the caution list around surgery and alongside anticoagulant or antiplatelet medication, by the same precautionary logic this library applies to nattokinase, and case reports of bleeding exist. Not for use in pregnancy on absent data.
