MitoQ
A clever targeting trick, a null flagship trial, one 20-person positive, and a premium price.

DoseTrials use 20 to 80 mg/day, a fraction of ordinary CoQ10 doses, because the targeting chemistry concentrates it in mitochondria several-hundred-fold in cell models. The concentration claim is well demonstrated in vitro; its translation into human tissue advantage over regular CoQ10 (graded C+ in this library) has never been shown head-to-head on an outcome.
BasisMitoQ earns credit for being genuinely trialed in humans, more than most premium longevity molecules can say.
MitoQ is coenzyme Q10 chemically tethered to a molecule that concentrates inside mitochondria, sold as a targeted antioxidant for aging, vessels, and energy. We grade MitoQ C- on evidence and D for worth-it at about $45-70 per month.
What it does
MitoQ is coenzyme Q10 chemically tethered to a molecule that concentrates inside mitochondria, sold as a targeted antioxidant for aging, vessels, and energy. It is the premium evolution of this library's CoQ10 record, and it is graded against what its own trials found, not against its parent compound's file.
What the research says
Improved a blood-vessel function measure in one small crossover trial in older adults
Evidence: emerging. Rossman 2018 randomized 20 healthy older adults to MitoQ 20 mg/day or placebo for six weeks in crossover: brachial artery flow-mediated dilation improved, with reduced oxidized LDL. A real trial with a surrogate endpoint and 20 participants: the definition of preliminary.
The disease trial was null, and exercise studies are mixed to negative
Evidence: moderate. In Parkinson's disease (Snow 2010), 128 patients took MitoQ for twelve months with no benefit over placebo on progression measures. Exercise studies since have found no performance benefit, with some suggestion of blunted training adaptation signals, consistent with the broader finding that high-dose antioxidants can interfere with exercise's own signaling (a theme this library's vitamin C and E discussions also carry).
MitoQ earns credit for being genuinely trialed in humans, more than most premium longevity molecules can say. The trials say: no in Parkinson's, a small surrogate yes in vessels, nothing for performance. Against a price several times its parent compound's, C- for the evidence and a worth-it grade below it for the economics.
Is it worth it?
Evidence (C-) sits below the leaderboard floor, so the worth-it grade is capped at C+; the derived value lands at D, because roughly $57 a month buys a mixed file that a far cheaper parent compound matches or beats. Derived from the October 2026 worth-it model.

Worth-it leaderboard
MitoQ sits outside the worth-it top 20.
MitoQ sits outside the current top 20 on the gated leaderboard. The leaders below clear an evidence floor of B- before price counts at all, so cheap alone never earns a rank.

Safety notes
Tolerability in trials is acceptable, with GI upset the common complaint. The theoretical concern with potent mitochondria-targeted antioxidants, interference with beneficial exercise signaling and with redox-dependent drug actions, is unresolved rather than established. No long-term safety data exist at consumer scale.

- 2026-10-23|Graded from the Parkinson's trial, the Rossman vascular crossover, and the exercise literature. MitoQ is the extension's clearest premium-pricing case: a clever molecule, a null flagship trial, one 20-person surrogate positive, and a supplement price set by the narrative.
- 2026-10-23|Not listed under any goal: a 20-person surrogate finding does not carry a goal tag.
