Huperzine A
A genuine dementia-drug mechanism sold to healthy people with no healthy-people evidence.

Typical cost
DoseDementia trials used 100 to 400 mcg twice daily. Nootropic products typically carry 50 to 200 mcg per capsule, often cycled on the theory that its long half-life (about 24 hours, with enzyme inhibition outlasting blood levels) accumulates; that cycling theory is itself untested. This is a drug-strength mechanism at microgram doses: more is not a casual experiment.
BasisHuperzine A is a real cholinesterase inhibitor, the same mechanism class as approved dementia drugs, and it should be graded like one: the supporting trials are small and quality-limited, the most rigorous trial missed, and there is no evidence whatsoever in the healthy population that buys most of it.
Huperzine A is an alkaloid from Chinese clubmoss (Huperzia serrata) that inhibits acetylcholinesterase, the enzyme that breaks down acetylcholine, the same target as prescription Alzheimer's drugs like donepezil. We grade Huperzine A C- on evidence and C- for worth-it at about $10-20 per month.
What it does
Huperzine A is an alkaloid from Chinese clubmoss (Huperzia serrata) that inhibits acetylcholinesterase, the enzyme that breaks down acetylcholine, the same target as prescription Alzheimer's drugs like donepezil. In China it is used as a drug for dementia; in the US it is sold as a supplement, mostly to healthy buyers as a nootropic.
What the research says
Small cognitive benefits in Alzheimer's disease across Chinese trials of uneven quality
Evidence: emerging. Meta-analyses (Yang 2013 and successors) pooling Chinese RCTs find huperzine A improved MMSE and daily-living scores versus control over 8 to 16 weeks in Alzheimer's patients. The trials are small, short, and at meaningful bias risk; effect sizes in such literatures typically shrink under independent replication.
The best Western trial missed its primary endpoint
Evidence: moderate. The US Alzheimer's Disease Cooperative Study trial (Rafii 2011, Neurology; 210 patients, 16 weeks) found no significant benefit on the ADAS-Cog at 200 mcg twice daily; the 400 mcg dose showed a nominal signal on that scale without a dose-response pattern to trust. No regulator outside China has approved huperzine A for dementia.
No evidence in healthy people, where most of it is sold
Evidence: weak. The nootropic market sells huperzine A to students and knowledge workers for focus and memory. There are no trials in healthy cognition. The entire evidence file is dementia patients, short-term, with a cholinesterase drug mechanism, and borrowing it for enhancement is borrowing a disease literature wholesale.
Huperzine A is a real cholinesterase inhibitor, the same mechanism class as approved dementia drugs, and it should be graded like one: the supporting trials are small and quality-limited, the most rigorous trial missed, and there is no evidence whatsoever in the healthy population that buys most of it. Mechanism plus mixed disease data plus zero data for its actual market lands at C-, with safety notes that matter more than the grade for this record.
Is it worth it?
Evidence (C-) sits below the leaderboard floor, so the worth-it grade is capped at C+ however inexpensive it is; the derived value lands at C-. About $15 a month at common doses. Derived from the October 2026 worth-it model.

Huperzine A sits outside the worth-it top 20.
Huperzine A sits outside the current top 20 on the gated leaderboard. The leaders below clear an evidence floor of B- before price counts at all, so cheap alone never earns a rank.

Safety notes
Cholinergic side effects are the profile: nausea, vomiting, diarrhea, sweating, blurred vision, and slowed heart rate; fainting is reported. Do not combine casually with prescription cholinesterase inhibitors (donepezil and relatives) or with anticholinergic drugs, whose effects it opposes; disclose it before anesthesia, as with other cholinesterase inhibitors. Avoid in pregnancy and in people with heart block, asthma, or seizure disorders unless a clinician directs otherwise. A supplement label does not make a drug mechanism gentle.
