Lithium Orotate
Fascinating neuroprotective signals at microdoses; the human proof is not there yet.
Lithium orotate is an over-the-counter form of lithium that delivers 1 to 5 mg of elemental lithium per day, a tiny fraction of the 100 to 300 mg used in prescription lithium carbonate for bipolar disorder. We grade Lithium Orotate C+ on evidence and C+ for worth-it at about $5-10 per month.

What it does
Lithium orotate is an over-the-counter form of lithium that delivers 1 to 5 mg of elemental lithium per day, a tiny fraction of the 100 to 300 mg used in prescription lithium carbonate for bipolar disorder. The orotate salt is claimed to cross the blood-brain barrier more efficiently, though direct human evidence for that claim is thin. The interest comes from population data linking trace lithium in drinking water to lower dementia and suicide rates.
What the research says
May support mood stability at microdoses
Evidence: emerging. A 2025 survey of 211 OTC lithium supplement users found 19 percent reported mood improvements and 20 percent reported anxiety improvements, but there was no placebo control. A small 1994 study of former drug users on 0.4 mg/day lithium from brewer's yeast reported improved mood scores. No well-powered placebo-controlled RCT has tested lithium orotate for mood.
May slow cognitive decline or dementia progression
Evidence: emerging. A 15-month RCT (Nunes et al., Journal of Alzheimer's Disease, 2013) found cognitive stabilization with microdose lithium (~300 mcg/day) in mild cognitive impairment. A 2025 Nature paper showed brain lithium deficiency accelerates Alzheimer's pathology in mice and that low-dose lithium orotate rescued memory. But a 2026 meta-analysis of 6 lithium RCTs in Alzheimer's dementia found no consistent cognitive or functional benefit.
Extends lifespan or healthspan
Evidence: weak. The longevity case is entirely extrapolated from population correlations (trace lithium in groundwater and lower mortality) and animal work. No human trial has tested lithium orotate against longevity or healthspan outcomes.
The biological plausibility is unusually strong for a C+ grade: lithium's effects on GSK-3, BDNF, and neuroprotection are well established pharmacologically, and the 2025 Nature paper on brain lithium deficiency is genuinely important. But the supplement-specific human record is observational surveys and one small RCT. C+ reflects a compelling mechanism with a thin human record.

Is it worth it?
Evidence (C+) sits below the leaderboard floor, so the worth-it grade is capped at C+ however inexpensive it is. About $7.5 a month at a common dose. Derived from the October 2026 worth-it model.

Dosage contextemerging
Typical OTC products provide 5 mg elemental lithium per capsule (from ~120 mg lithium orotate). Research doses range from 0.3 mg to 5 mg/day elemental lithium. No established therapeutic dose exists for supplementation.
Safety notes
Do not confuse with prescription lithium carbonate: doses differ by 50x or more. Avoid during pregnancy and with kidney or thyroid disease. Can interact with diuretics, NSAIDs, and ACE inhibitors. Long-term human safety data at supplement doses are limited. Discuss with a clinician before starting.
